About the role

Description

Cheektowaga / Erie County, NY | Clinical Services

Build trust. Support recovery. Help people live more independently.

At Venture Forthe, meaningful care starts with listening, showing up consistently, and respecting each person’s choices. We’re looking for a compassionate, dependable full-time Community Integration Mentor who wants to build strong client relationships and help individuals make lasting progress in their everyday lives.

If you take pride in following through, meeting people where they are, and helping them work toward goals that matter to them, we encourage you to apply.

Your Role

You’ll work directly with individuals in their homes and communities through our Community Oriented Recovery and Empowerment Services (CORE) programs under the Office of Mental Health.

Your work will help clients strengthen coping skills, build relationships, navigate challenges, and gain greater independence. Progress may mean establishing a daily routine, accessing stable housing, reconnecting with others, or taking a step toward education. You’ll help clients recognize their strengths and put them into practice.

This full-time role calls for consistent engagement, thoughtful communication, and a commitment to quality care—including accurate documentation and collaboration with your clinical supervisor.

How You’ll Support Clients

  • Build trusting, respectful relationships that honor each client’s goals, preferences, and dignity.
  • Provide CORE services in clients’ homes and community settings.
  • Help clients develop coping strategies, adaptive behaviors, and relapse prevention skills.
  • Support access to community resources, including SNAP, housing, and mental health services.
  • Strengthen communication, social skills, and relationship-building.
  • Help clients practice independent living skills such as meal planning, budgeting, and maintaining daily routines.
  • Facilitate recovery-oriented activities that support long-term stability and independence.
  • Maintain regular communication with your clinical supervisor about client progress, challenges, and support needs.
  • Complete timely, accurate documentation that meets program and compliance standards.
  • Perform additional duties that support client care and program needs.
